Form Name Purpose
Application Form - Duplicate Nevada Certificate of Title To request a replacement of a Lost or Destroyed Title. The Owner mustbe able to prove ownership. Remember to make a copy of your Title and keep it in a separate location. Do not keep your Title in your Vehicle.
Application for Duplicate Registration Certificate To obtain a replacement copy of one's Vehicle Registration. Owner needs proof of ownership of vehicle, and information regarding the Vehicle.
Bill of Sale A Bill of Sale releases interest in a vehicle. A Certificate of Title or other ownership documents must accompany this form.
Change of Address Application Request to change a Driver's address. A separate form is needed for each driver or vehicle owner.
Delivery Affidavit Used when a vehicle of any kind is delivered and received in the State of Nevada. For example, an RV is purchased in California, but the Purchaser is a resident of Nevada, he/she accepts delivery of the vehicle in Nevada.
Disabled License Plates Application and/or Placards For first time applications for a Disabled Person's license plate or motor cycle sticker. Application must be made in person.
Driver License Application (Non-Commercial) Complete an application for a Driver License.Normally for the first time.
Erasure Affidavit To correct a legitimate error or change made on a DMV Form. The individual - signing before a notary public, is taking full responsibility for an error, change, - or removal of information on ownership documents. Owner must complete and - sign the Erasure Affidavit. If a person is being removed from the ownership documents, that individual is required to sign the affidavit also. When - representing a business, the business name and the name of the - authorized business representative must appear on the affidavit.
Family Sale/Gift Affidavit A Power of Attorney making a person your Attorney-in-Fact, giving him/her - power to represent you at the DMV to negotiate business concerning the vehicle - mentioned on the Power of Attorney of the Motor Vehicle.
